Activities Approval Form

Due to a new DISD policy, all parents must now sign the Student Activity Approval Form in addition to the field trip forms previously signed during the choir parent meeting.

Please print the first page of the attached form, sign it, and turn it in to Mrs. Stephens in the choir office on Monday or email it to [email protected] by monday. Thanks for your cooperation!
